After a cagy opening Chelsea opened the scoring with a classic counter attack.
N'Golo Kante gave the ball to Hazard, and he played a one-two with Pedro before rounding Darren Randolph and firing home.
The Blues should have been two up before half-time when Pedro volleyed from 18-yards out, but Randolph produced a fine diving save.
After the break Chelsea got their second when Hazard's corner was flicked into the patch of Costa by Pedro Obiang, and the Blues striker kneed the ball home.
West Ham almost got back in it when Sofiane Feghouli's shot was stopped by Thibaut Courtois, before Jose Fonte headed over.
Chelsea almost added a third when Costa struck from distance, but Randolph produced another fine save.
Manuel Lanzini grabbed a late consolation for the Hammers but the Blues held on for victory and tightend their grip on top spot in the Premier League table.
